Past Care Letter

Provides an outline of the past care and support which would have been reasonably necessary, and describes (from precognitions and/or from information gained at interview) the broad picture of what family and others say they did by way of care & support.

Future Needs Report

Provides an outline of the past care and support which would have been reasonably necessary, and describes (from precognitions and/or from information gained at interview) the broad picture of what family and others say they did by way of care & support.

Immediate Needs Assessment (INA)

INA’s are instructed under the Rehabilitation Code and are generally requested early post injury. They are intended to identify a series of short to mid-term recommendations to facilitate the early stages of recovery and rehabilitation. This may include the provision of case management, care, specialist equipment, treatment and housing.

Future Care Report

These reports set out current and future care and case management needs, and extra living expenses. Therapies, equipment and accommodation may be commented on, but not in any detail.

Loss of Services Report

The aim of a Loss of Services Report is to outline and cost what extra care and services is reasonably necessary in lieu of the deceased. The reports can also highlight future services and associated costs that would be necessary to return the family members to as close to the pre-accident position in terms of childcare/lifestyle/work etc.

Mesothelioma/Asbestos Related Care Reports

This report outlines the needs for individuals diagnosed with mesothelioma due to asbestos exposure. It includes detailed plans for treatments, assistive devices, home modifications, personal care, and psychological support to ensure the patient's quality of life.

End of Life / Palliative Care Report

These reports set out current and future needs (taking into account life expectancy and treatment). They take into account statutory provision availability and client choice. The report includes care, case management, equipment/adaptations, and extra living expenses. These reports are prepared by a Registered General Nurse experienced in end-of-life care.

Nursing Only

Liability Reports/ Duty of Care Reports

These are desktop reports prepared by a Registered General Nurse dealing with potential negligence within nursing care provision (community-based, hospital-based or care home-based). Current legislation and best practice are referenced to guide the reporter’s clinical judgement.

Equipment Report

These set out the main items of equipment that may be required by the injured party together with any occupational therapy needs. These may be requested at the early stages of rehabilitation to apply for appropriate funding/interim payments.

Accommodation Reports

Accommodation Reports

This report assesses the housing needs of an individual who has suffered an injury or illness. It includes recommendations for modifications to their current home or the need for new accommodation to ensure accessibility and safety.

Case Management

These set out, for litigation purposes, the long term predicted case management needs of the injured party, including rehabilitation and therapy.

Neuropsychology

Neuropsychology

Neuropsychology assessment is appropriate where there is actual or suspected brain injury resulting in changes in cognitive, emotional and behavioural functioning.

Clinical Psychology

Clinical psychology assessment is appropriate where there are emotional consequences of injuries e.g. anxiety, depression, Post Traumatic Stress Disorder (PTSD) and adjustment disorder.

Occupational Therapy Reports

This report provides a detailed assessment of an individual's functional difficulties and needs following an injury or illness. It includes recommendations for therapies, assistive devices, and home modifications to optimise the individual's independence and quality of life.

Physiotherapy Reports

A physiotherapy report provides a detailed assessment of an individual's physical impairments and rehabilitation needs following an injury or illness. It includes recommendations for treatment, exercises, and assistive devices to enhance recovery and improve functional abilities.

Referral Process & Requirements

Referrals are welcomed from legal professionals, trustees, and personal injury solicitors. Individuals and their families can also self-refer. GPs, hospital doctors, and other healthcare professionals may also initiate referrals, with NHS referrals requiring an Extra Contractual Referral (ECR). Referrals from legal professionals can be particularly advantageous in personal injury litigation.
