Our Case Management Service provides expert guidance and support to help navigate this difficult journey and rebuild lives.

We offer comprehensive, personalised support, connecting clients with a network of specialists, including psychologists, physiotherapists, occupational therapists, and speech and language therapists. We navigate the complexities of the healthcare system on their behalf, set up and coordinate care packages and develop tailored rehabilitation plans focused on maximising physical and cognitive recovery.

Our goal-directed case management, combined with community-based rehabilitation (therapy and support services delivered in the client's home and community), empowers clients to regain independence, pursue new interests, and reconnect with their communities. We understand the emotional toll a serious injury can take and provide compassionate support to both individuals and their families. What is a Case Manager, and how can they help?

A case manager acts as a central point of contact and expert guide for individuals and families navigating the complexities of recovery after a serious injury. They provide comprehensive support, coordinating all aspects of rehabilitation and care to achieve the best possible long-term outcomes.

Working with clients who have experienced a range of challenges, including brain injury, spinal injury, orthopaedic injuries, and other complex injuries, a case manager:

Assesses needs: They conduct thorough assessments to understand the individual's unique physical, cognitive, social, and emotional needs.

Develops a plan: They create a personalised rehabilitation plan in collaboration with the client, their family, and a team of healthcare professionals.

Coordinates care: They connect clients with appropriate specialists, manage appointments, and ensure seamless communication between all involved parties.

Identifies resources: They locate and secure access to local resources, support services, and activities to aid recovery and community reintegration.

Monitors progress: They regularly review progress, adjusting the plan as needed to ensure goals are being met.

Advocates for clients: They act as a client's advocate, ensuring their voice is heard and their needs are met.

Organising Community Based Rehabilitation (CBR)

CBR offers significant advantages for many, either as a primary treatment or following inpatient care. This intensive, goal-focused approach empowers clients to recover in the comfort of their own home.

Set Up & Manage
Care Package

A Case Manager will ensure that an appropriate care package is in place and is routinely reviewed. They will be involved at all stages of the process including staff recruitment, training and monitoring.

Establishing Structure and
Routine

Case managers help clients explore leisure, recreational, vocational, and educational opportunities, supporting them through practical aspects of rehabilitation like budgeting, travel, planning, and fatigue management.

Coordinating a multidisciplinary assessment and treatment plan

This may include promoting independent travel, budgeting, social skills, planning and organisation and safety awareness.

Family Intervention

A Case Manager can support a family in a number of ways. This may include the provision of education, liaison between the family and external agencies and help to reduce stress and improve coping.

Training

Our Case Managers provide bespoke education, training and support to families, carers, care/ support staff and other professionals. 

Referral Process & Case Management Pathway

Referrals are welcomed from legal professionals, trustees, and personal injury solicitors. Individuals and their families can also self-refer. GPs, hospital doctors, and other healthcare professionals may also initiate referrals, with NHS referrals requiring an Extra Contractual Referral (ECR). Referrals from legal professionals can be particularly advantageous in personal injury litigation.

Our case managers are registered with the British Association of Brain Injury Case Managers (BABICM). We also have links with CMSUK another organisation specifically geared towards developing case management in the UK.

Make a Referral
