We provide community based rehabilitation primarily within the client’s homes and this involves close collaboration with the client’s family, therapy team and any other services who may be involved.
Key Responsibilities:
• Assessing needs
• Formulating care and rehabilitation plans
• Goal setting
• Coordinating services and signposting to other services as required
• Assessing risk and evaluating input.
• Writing Immediate Needs Assessment reports. These outline an individual’s rehabilitation, care/support, housing and equipment needs and are often instructed under the Rehab Code. Training regarding assessment and report writing will be provided.
Skills and experience required:
• Applicants must be HCPC registered and have a clinical qualification such as Occupational Therapy, Physiotherapy, Speech and Language Therapist, Nursing or the equivalent.
• The successful candidate must have experience of collaborative working within a multidisciplinary team and with clients and their families.
• Experience of case management and preparing medico-legal reports is desirable but not essential.
• Strong interpersonal and communication skills.
• The ability to think creatively and adapt approach when required.
• Driver with access to own car.
• Flexibility and the willingness to travel to client’s homes both locally and considerably further afield.
